Medical Records

Medical records are kept by the Student Health Center for seven years following graduation or from the time the student left the college. In order to obtain a copy of a medical record, please call 401-865-2422 or fax request to the Student Health Center at 401-865-2809.

The request should include the following information:

Your name and birth date
Today’s date
What specific information you are requesting
Where you would like the information sent
The year you graduated or left the college
Your phone number and address
Your signature

Office Visits: By appointment only during the academic year. The Health Center is closed during Thanksgiving Break, Christmas Break, Easter Break, Spring Break, and the summer.
