Health Records
Copies of health records and immunizations can be requested in writing from the health center at any time. In order to request, please provide a SIGNED document that states name, year of graduation, date of birth, and where you would like this information sent. Records are kept on file for seven years after graduation. Please allow three weeks to process during the summer break.

Office Visits: By appointment only during the academic year. The Health Center is closed during Thanksgiving Break, Christmas Break, Easter Break, Spring Break, and the summer.
