Graduate Student Health Records & Immunization Requirements

The Rhode Island Department of Health (RIDOH) requires that Providence College keep certain medical information on file for all full-time graduate students (graduate students taking 9 credits or more).

Required Medical Information – Graduate Students

The Graduate Student Immunization and TB Form must be submitted to the Providence College Student Health Center by August 15th. Per the Rhode Island Department of Health, students who have not submitted documentation for the required immunizations and TB questionnaire will NOT be permitted on campus.

Questions about your form?

We will be reviewing records in the order received throughout the summer. There is no need to email or call the Health Center to check on the status of your form. The Health Center will reach out to you if there are questions or issues with your information.
